This a project proposal; I have attached a literature review sample. I need to rewrite the literature review which also includes the framework for the project. It is a project in the level of documentation compliance in the multidisciplinary team. the gold standards for clinical documentation is 90% compliance (mandated by The Joint Commission [TJC]), the home health agency currently is at 78%. the practitioners that document in the electronic health records (EHR) is the multidisciplinary team (MDT): Nurse, speech therapist, physical therapist, social worker.

I am trying to evaluate their documentation in home health patients and evaluate how far is their documentation from the 90% practice standards, report findings and write recommendations for improvement. It would be best to introduce in the lit review what is the documentation compliance in home health in the US and then in California and locally in Los Angeles. The lit review needs to emphasize the importance of clinical audit in home health and Why and how the studies support the project on chart review in home health documentation. PICOT : (P) In the Multidisciplinary Team (MDT) at a Home Health agency (I) Implementing a Retrospective Chart Review (RCR) to evaluate documentation compliance (C) Compared to TJC/CMS practice standards (90% internal agency target, not a national standard) (O) will identify gaps in documentation and provide recommendations (to decrease gaps in practice) (T) over 12 weeks.

I am trying to conduct a retrospective chart review in a home health agency, using the agency Audit tool (AT) that was used for the TJC visit 3 years ago. using a mix of charts for the agency I want to collect data from charts and see patterns of poor documentation in the MDT, the mock survey revealed that the documentation compliance is at 78% and their internal target is 90% based on the TJC and CMS. and they need to follow the CMS conditions of participation to obtain accreditation for 2021, meaning increase from 78% to 90%.

I have posted the guidelines for the lit review, key points of the project, some articles, the framework (follow the sample lit review -including the framework i will use for the project), I would like you to help me write the LITERATURE REVIEW including the framework that I will use (CP-FIT). Clinical Performance Feedback Intervention Theory (CP-FIT) is the framework I want to use, but if you think about another framework that applies to the retrospective chart review (RCR) i am conducting, would be fine. let me know if you have questions. i am sending you some articles, you can add your own articles, make sure the references are not too old, 5-7 years are acceptable. meaning 2015-2021 is acceptable.
